These forms are required for each child prior to the first day of the school year.
This form needs to be completed by your child’s doctor and the physical exam needs to be completed within the past 12 months. Record of current immunizations should also accompany the physical form.
Clearances are required for any adult wishing to volunteer at the preschool. This includes classroom/holiday parties and field trips. Please use the links below to obtain your PA clearances.
